Moving from family home into shared supported living

A young adult in Liverpool transitioned from full-time family care into a shared supported living arrangement, with daily living skills built up gradually alongside consistent overnight support.

The situation

A participant in their early twenties had lived with family in Liverpool their whole life. As their family's own circumstances changed, both the participant and their family started exploring what a shared supported living arrangement might look like, without rushing the decision.

The support we provided

LYD NDIS worked with the participant's Support Coordinator to plan a 12-week transition: starting with short overnight stays to build comfort with the new environment, then gradually increasing to full-time shared living with consistent overnight support workers.

The outcome

The participant now lives independently in shared supported accommodation with overnight LYD NDIS support, and visits family on weekends rather than living with them full-time. Family report feeling confident in the arrangement because the transition was gradual and consistently supported.
