The situation
A participant in their early twenties had lived with family in Liverpool their whole life. As their family's own circumstances changed, both the participant and their family started exploring what a shared supported living arrangement might look like, without rushing the decision.
The support we provided
LYD NDIS worked with the participant's Support Coordinator to plan a 12-week transition: starting with short overnight stays to build comfort with the new environment, then gradually increasing to full-time shared living with consistent overnight support workers.
- Weekly transition planning meetings with family and Support Coordinator
- Gradual increase in overnight stays over 12 weeks
- Daily living skills coaching alongside overnight support
- Consistent support worker pairing to build trust and routine
The outcome
The participant now lives independently in shared supported accommodation with overnight LYD NDIS support, and visits family on weekends rather than living with them full-time. Family report feeling confident in the arrangement because the transition was gradual and consistently supported.
